DC Comic Questionnaire Insert Variant. This variant of Black Lighting #8 contains a 4-page, 6.75-in. x 8.5-in., bound in DC customer survey form. Otherwise, the comic is identical to ordinary copies. No info is available as to how many copies of this issue or other May cover-dated issues contained the survey.
